LIABILITY & RELEASE WAIVER

For the purposes of this waiver, {name} and or Parent/Legal Guardian shall both be henceforth referred to as "The Member".

The Member knowingly and without duress, do voluntarily submit their entry to 1664549 ONTARIO INC o/a SHAOLIN Martial Arts Canada to receive martial arts instruction. In consideration of 1664549 ONTARIO INC o/a SHAOLIN Martial Arts Canada accepting their application, The Member hereby assume all risk of physical and mental injuries, disabilities and losses which may result from or in connection with their participation in 1664549 ONTARIO INC o/a SHAOLIN Martial Arts Canada. Acting for The Member's, heirs, personal officers, agents, representatives and assignees, The Member does hereby release 1664549 ONTARIO INC o/a SHAOLIN Martial Arts Canada, its Members, officers, agents, representatives, and assignees from all claims, actions, suits, and controversies at law or in equity by reason of any matter, cause or thing whatsoever that The Member may sustain as a result of or in connection with their participation in the course of instruction at 1664549 ONTARIO INC o/a SHAOLIN Martial Arts Canada.

The Member agrees to abide by and follow the Rules established by 1664549 ONTARIO INC o/a SHAOLIN Martial Arts Canada.

The Member agrees that their performance, attendance, and participation in 1664549 ONTARIO INC o/a SHAOLIN Martial Arts Canada may be filmed or otherwise recorded during the course of training.

The Member consents to 1664549 ONTARIO INC o/a SHAOLIN Martial Arts Canada using The Member's name, voice, poses, pictures and biographical data, in any form or language, with or without other material, throughout the world, without limitation, for television, radio, video, theatrical medium picture, or any other medium by any devices now known or hereafter devised, and I do hereby waive any compensation in regard thereof as well as any future rights to the aforementioned.

The Member has read and fully understands the waiver listed above.

If the Member is under 18 years of age:
